ON-LINE REPEAT PRESCRIPTION REQUESTS

There are currently two ways to order a repeat prescription either:

(a) If you are a patient aged 16 years or over by using our internet booking service.  Once registered for the internet service you can also book routine appointments and access you medical record as well as repeat prescriptions using this facility.

If you haven't registered yet click here for more information. You can download the form to complete here and you will then need to bring this in to our reception desk to sign up for the service. You will need to do this yourself in person and bring with you some form of ID such as photo driving licence or passport.

Click on the blue button link below, only IF YOU HAVE ALREADY REGISTERED AS PER THE INSTRUCTIONS ABOVE, to order a repeat prescription.

When requesting prescriptions please allow 3 working days, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.

THIS OPTION WILL BE REMOVED FROM THE WEBSITE AS OF 31/7/18. IF YOU WISH TO ORDER PRESCRIPTIONS VIA AN ONLINE METHOD YOU WILL NEED TO SIGN UP FOR ONLINE SERVICES AS PER OPTION A.

You can request repeat medication which your doctor has already set up for you on the surgery computer using the form below.

  • We cannot accept requests for any medication which is not on your repeat medication slip.
  • The prescriptions will be sent to the pharmacy which you have chosen in the past.
  • Prescription requests will usually be processed as soon as they are received, however, we ask you to allow 3 working days for processing and for them to be at the pharmacy to avoid disappointment.

You may find it easier if you have the slip from your last prescription to hand. Please list only the items you require this time. We ask you to state strengths to avoid confusion when two strengths of the same medication are used. Please include a telephone number at which you can be contacted during the day in case we need to contact you.

Please note that this form is sent over standard e-mail when transferring your message to the surgery. The data is not sent through a secure channel and could, possibly, be viewed by third parties.
