Jennie is also a GP at Honiton Surgery. She provides strategic and clinical leadership amongst our teams. Her PCN working day is every Wednesday. |

HOSMS PCN Deputy Manager is Charlotte Nally |
|
Charlotte is also the Deputy Manager at Honiton Surgery. She works helping to advance our PCN through leading, managing and coordinating all our teams within the PCN. Charlotte is based at Honiton Surgery. Monday, Wednesday, Thursday 8am – 5:30pm and Tuesday 8am – 1pm. |

Emma Vigurs is the Operations Manager at Sid Valley Practice |

Emma is responsible for leading and managing the practice and implementing any changes agreed as a PCN. Tuesday, Friday 8am – 6pm and Wednesday,Thursday 8am – 5:30pm. |
PCN Pharmacy Team: |
|
Pharmacy Technicians can provide support with; medication compliance issues, Care Home ordering, complex medication requests, formulary medication, finding cost-effective medications, covert administration, liquid medication/ swallowing difficulties, pharmacy out of stocks/alternative medication queries and provide support with the implementation of national prescribing policies and guidance. Clinical pharmacists complete structured medication reviews as well as providing support with complex medication titrations, medication interactions, medication side effects and pharmacokinetics of medications. |

Zoe works across all 3 practices. Zoe is based at Honiton Surgery on Monday and Thursdays, Coleridge Medical Centre on Tuesdays and Sid Valley Practice on Wednesdays and Fridays 9am - 5pm |

Ryan Allman is the Senior Pharmacy Technician |

Ryan leads the team to provide care for patients registered at all 3 practices. Ryan is based at Honiton Surgery on Mondays and Thursdays, Coleridge Medical Centre on Tuesdays, Sid Valley Practice on Wednesdays and works from home on a Friday. 8:30am – 4:30pm. |

Rachael Stewart is the Pharmacy Technician for Honiton Surgery |

Rachael is based at Honiton Surgery on Tuesdays and Thursdays, and alternate Fridays at Coleridge Medical Centre or Honiton Surgery. She works form home on Mondays. 8:30am – 4:30pm. |
PCN Care Home Team: |
|
The Care Home Team provide an acute service for all care home patients across the 3 towns as well as managing frailty, dementia, and other health concerns. Every care home receives a weekly ward round, whereby routine queries are discussed and then managed within the team. New residents get a holistic assessment by the OT and a full review by the GP. Bev Watkins is the dedicated GP for the Care Home Team |

Bev provides support to all residents across the three towns. She will also be providing clinical advice & supporting the PCN Care Home Team. Monday, Tuesday, Wednesday and Friday 9am – 6pm. |

Lucy Salmon is the OT Clinical Lead for the Care Home Team |
|
Lucy provides EHCH across all 3 practices, as well as out in our Care Homes. Monday to Friday 8am – 4pm between Honiton Surgery, Sid Valley Practice & Coleridge Medical Centre. |

Amy Lee is a PCN OT |

Amy will be working with the Care Home Team across all 3 practices. Monday 8:30am - 5pm, Tuesdays and Wednesdays 9:30 - 5pm between Honiton Surgery, Sid Valley Practice and Coleridge Medical Centre, as well as in our Care Homes. |

Alex Hooper is a PCN Paramedic |

Alex will be working with the Care Home team. He is based predominantly in Sid Valley Practice but will also be working remotely in all three areas. Monday, Tuesday, Thursday and Friday 8am – 6pm. |

Isaac will be working with the Care Home. He is based predominantly in Sid Valley Practice but will also be working remotely in all three area’s. Monday to Thursday 8am – 6pm. |

Chloe Hopwood is a Care Home Coordinator

Chloe provides support to Care Home staff and residents across all 3 practices. She is based at Sid Valley Practice. Monday to Wednesday 9am – 5pm and Thursday to Friday 8:45am – 4:45pm. |

Danni Diamond is a Care Home Coordinator |

Danni provides support to Care Home staff and residents across all 3 practices. She is based at Sid Valley Practice. Monday to Friday 8:15am – 4:15pm. |
PCN MSK Team: |
|
The MSK specialists are first contact practitioners, they therefore predominantly see patients booked in directly by the surgery staff and GP’s at each practice. All team members are specialist Physiotherapists working at an advanced level and some are also experienced in injection therapy. |
